Being a disqualified student means that you did not maintain the minimum GPA requirements set by the university. In order to come back you need to complete the following steps:

  1. Complete a formal application for readmission through CSUMentor
  2. Send official transcripts of course(s) completed at another college of university during the period in which you were disqualified, and 
  3. Complete the “Previously Disqualified Student Questionnaire” which is found on the Student Forms site. 

The process of readmission goes through the Student Services Center/EOP office for the College of Health and Human Development, so please contact that office for further instructions. They are located in Sequoia Hall 111, and the number is 818-677-2883.
