I am a previously disqualified student, how do I come back?
Being a disqualified student means that you did not maintain the minimum GPA requirements set by the university. In order to come back you need to complete the following steps:
- Complete a formal application for readmission through CSUMentor
- Send official transcripts of course(s) completed at another college of university during the period in which you were disqualified, and
- Complete the “Previously Disqualified Student Questionnaire” which is found on the Student Forms site.
The process of readmission goes through the Student Services Center/EOP office for the College of Health and Human Development, so please contact that office for further instructions. They are located in Sequoia Hall 111, and the number is 818-677-2883.
